Your existing medical records can still be accessed online at Hawaii Pacific Health MyChart.

Note: if you are transitioning care to Queens, there is no need to transfer records.

If you receive a bill after 4/1/2024 and you ARE NOT engaged with case management, please pay your account balance to ensure that you will not be charged additional fees.
